Alec Baldwin ignored the golden rule of gun safety by pointing the prop at someone but the gun never should have been loaded with live ammunition to begin with, experts say. 

Zak Knight, a pyrotechnic and special effects engineer who is a member of Local 44, told DailyMail.com on Friday: 'There should have never been live rounds on a movie set, that's number one. 

Number two is every single person on a movie set has a right to inspect a weapon before it's fired. 

And number three is, there is no reason to ever put a person in front of a weapon that's firing. 

'Anytime you see a movie where the barrel is pointed down the camera lens, there should not be an operator behind it. 

It's obvious that the considerations of this resulted in that gun being pointed directly at two people.

A United Airlines flight bound for Hawaii was forced to make an emergency landing shortly after taking off from Denver after suffering major damage to one of its engines 15,000ft above ground and dropping huge pieces of debris over a neighborhood in Colorado as dramatic audio from the mayday call reveals the pilot asking air traffic control to immediately turn the aircraft around. 

Video recorded by passengers aboard Flight UA328, which was carrying 231 travelers and 10 crew members, showed the engine on fire shortly after takeoff from Denver. 

There were no reported injuries either on board or on the ground. One homeowner in the Denver suburb of Broomfield, Colorado said they were making a sandwich in their kitchen when debris crashed through their roof and landed just two feet away, according to KMGH-TV.

Warzone/The Drive: Code Brown As This F-4 Phantom Nearly Collides With A Mosque's Minaret 

The head-up display footage is a reminder of just how fast things can go from calm to terrifying in the fighter flying business.

Flying fighter aircraft at low-level may be one of the world's ultimate e-ticket rides, but it is not without its fair share of dangers. Case in point, the video below, which I first saw on Twitter a couple of months ago, but didn't save it. I finally hunted it down again to share it with our readers. 

From what I can piece together, it shows the head-up display (HUD) footage from an upgraded Turkish F-4 Phantom. All seems to be well until seemingly out of nowhere a large mosque appears. The jet pitches up at the very last moment, just narrowly missing its towering minarets, one of which was dead-on in the line of flight of the Turkish Phantom. It is truly a terrifying moment to behold, although the crew's reaction was astonishingly lighthearted.

Three National Guard Pilots Killed In Helicopter Crash Near Boise, Idaho

 

Daily Mail: Three National Guard pilots are killed in helicopter crash in Idaho after their aircraft went down in snowy weather during training flight 

 * The Idaho National Guard said the three personnel were participating in a routine training flight when they were killed south of Lucky Peak near Boise 

 * They were flying in a UH-60 Black Hawk helicopter when it went down Tuesday 

 * Col. Christopher Burt said the aircraft was last contacted at 7:45pm 

 * 'At approximately 12:15 a.m. Wednesday morning, crews located the aircraft and personnel,' the Idaho National Guard said in a statement 

 * The cause of the accident is unknown and an investigation will be launched

 * The identities of those killed has not yet been released 

Three National Guard pilots have been killed in helicopter crash in Idaho. 

They were flying in a UH-60 Black Hawk helicopter when it went down Tuesday evening south of Lucky Peak near Boise.

One cadet was killed and 19 others were injured on Thursday after an armored personnel carrier overturned during a training exercise at West Point Military Academy. Two active-service soldiers on the carrier were also hurt.

During a brief press conference held this afternoon, Lt. Gen. Darryl Williams, the superintendent of US Military Academy at West Point, revealed that a light medium tactical vehicle carrying a total of 20 cadets and two soldiers was headed to a land navigation training site as part of routine exercise when the truck flipped over.

Last Thursday afternoon, commuters driving down the 215 Freeway adjacent to Riverside County, California's March Air Reserve Base witnessed an incredible sight.

A pilot was forced to eject from his F-16 Fighting Falcon carrying live ordnance over the highway, deploying his chute as the fighter careened into the roof of a nearby warehouse.

* SEAL 1st Class Remington J. Peters struck the Hudson in May of 2017 following back-to-back malfunctions.
* Peters' main parachute deployed with a line-twist malfunction, which he spent too much time trying to untangle.
* The malfunctions caused Peters to free fall and prevented the deployment of his reserve chute.
* Peters had performed in over 100 Leap Frog shows, and completed over 600 jumps with his team, as well as 300 civilian jumps.

A twist in a main parachute line, and an additional entanglement that prevented the deployment of a reserve parachute system at the correct altitude and time, caused a Navy SEAL to fall to his death in the Hudson River off New York City last year, according to investigation documents.

Special Warfare Operator (SEAL) 1st Class Remington J. Peters struck the Hudson near Liberty State Park, NJ, during New York Fleet Week on May 28, 2017 following back-to-back malfunctions that caused him to free fall invertedly and hampered successful deployment of his reserve chute, according to a command-directed investigation obtained by Military.com through the Freedom of Information Act.

A chartered train carrying Republican lawmakers to a retreat collided with a trash truck Wednesday morning and left at least one person dead.

White House Press Secretary Sarah Huckabee Sanders there was one confirmed fatality and one person seriously injured.

"There are no serious injuries among members of Congress or their staff," she said in a statement.

The victim's identity was not immediately released.

Guided-missile cruiser USS Lake Champlain (CG-57) and a South Korean fishing vessel collided in international waters east of the Korean Peninsula, the Navy said in a statement this morning. That fishing vessel lacked functioning radio or GPS and did not heed audio warnings, a defense official told USNI News.

Lake Champlain was conducting routine operations at the time of the collision with the 65-foot boat, 502 Nam Yang. No one was injured, and both ships were able to navigate under their own power afterwards.

OSAN AIR BASE, South Korea -- The Army has released the final images taken by combat photographer Spc. Hilda Clayton moments before she died during a live-fire training accident in Afghanistan nearly four years ago.

The photos, which have been published in the May-June edition of the Army's Military Review journal, show the moment a mortar tube exploded, killing Clayton and four Afghan soldiers on July 2, 2013, in the eastern Afghan area of Qaraghahi.

Clayton, a visual information specialist attached to the 4th Armored Brigade Combat Team, 1st Cavalry Division based at Forward Operating Base Gamberi, was part of a training mission certifying Afghan forces on mortar operations.

Air Force Times: Airman dies in non-combat related incident in Southwest Asia

An airman died on Tuesday in a non-combat related incident while deployed in support of Operation Inherent Resolve, the Air Force and Defense Department announced Thursday.

Staff Sgt. Alexandria Mae Morrow, 25, was injured while performing maintenance duties during non-combat operations with the 332nd Air Expeditionary Wing in Southwest Asia, according to a statement from Mountain Home Air Force Base in Idaho.

Morrow, who was from Dansville, New York, was assigned to the 366th Aircraft Maintenance Squadron at Mountain Home Air Force Base, but she worked for the 322nd Air Expeditionary Wing overseas, according to a spokesman at Mountain Home Air Force Base.

ALAMOGORDO, N.M. — A civilian contractor was killed and an airman was injured in a training accident at a military range near Holloman Air Force Base in southern New Mexico Tuesday.

Capt. Bryant Davis, chief of public affairs for the 49th Wing at Holloman, said the two were part of a ground control party that was helping coordinate a live-fire training exercise at the Red Rio range, which is in the White Sands Missile Range complex. Davis said officials expect to release more information on the airman and contractor later on Wednesday.

An Army transport truck carrying tank ammunition skidded off a snowy road in Poland on Saturday, injuring two U.S. soldiers.

A preliminary investigation indicated that the truck was traveling too fast for the winter road conditions, and the driver lost control and slid off the road, according to a Polish Defense Ministry communique.

Some of the M-1 ammunition for M-1 Abrams tanks the truck was hauling spilled onto the road leading to Zagan, where several hundred U.S. troops and equipment for an American armored brigade are based.

Human Error To Blame For Cable Break On The USS Eisenhower Flight Deck That Injured Eight Sailors (Video)

Virginia Pilot: Navy: Human error to blame for March cable break aboard USS Eisenhower flight deck

Navy investigators blamed human error and an improperly programmed valve for a March incident in which eight sailors were injured when a cable used to catch a landing E-2C Hawkeye snapped on the flight deck of the USS Dwight D. Eisenhower.

According to a Navy report obtained by The Virginian-Pilot through a Freedom of Information Act request, maintenance personnel missed at least one and possibly two “critical steps” while working on an engine that helps operate the carrier flight deck’s cables, which are called cross deck pendants, after a previous landing. As a result, the engine failed to slow the aircraft, instead causing the pendant to break “at or near” the Hawkeye’s tailhook.

It was raining Humvees last week when a US Army training exercise at Hohenfels military base in Germany went wrong. Three military vehicles detached from their parachute systems and crashed violently to the ground.

An airborne drill at Hohenfels – the main headquarters of the US Army’s Joint Multinational Readiness Center – went wrong when C-130 Hercules tactical airlifters began to put HMMWV (Humvee) military off-roaders on the ground.

As a result, three improperly-fastened vehicles became three heaps of deformed metal.

8 U.S. Sailors Injured On The Aircraft Carrier USS Harry Truman When The Landing Cable Snapped

The Virginia Pilot:Navy: 8 sailors aboard USS Eisenhower injured when cable breaks during E-2C Hawkeye landing 

Eight sailors working on the flight deck of the USS Dwight D. Eisenhower were injured Friday when a cable used to catch a landing aircraft’s tailhook broke, according to the Navy.

Four of the sailors were flown by helicopter to Sentara Norfolk General Hospital with injuries that weren’t life-threatening, while two were taken to Portsmouth Naval Medical Center. Norfolk General has the only Level 1 trauma center in the region and handles the most severe injuries. When reached Friday night, Mike Maus, a Naval Air Force Atlantic spokesman, said the injured sailors were stable.

A U.S. Air Force F-16 Fighting Falcon jet crashed over northwestern Arizona on Thursday morning. The condition of the pilot was not immediately known.

A preliminary investigation has already been set up to find out what happened to the jet, which took off earlier that morning from 56th Fighter Wing at Luke Air Force Base.

"Luke AFB officials are working closely with local authorities in a search and rescue operation," Luke said in a news release Thursday afternoon. "Due to the remote location and rugged terrain, the status of the pilot is unknown."

US Coast Guard Suspends Search For 12 Missing Marines

Marine Times: Search for 12 Marines ends without finding survivors after helicopters crash

After five days without finding any survivors, the Coast Guard suspended its search for 12 Marines who have been missing since Jan. 14 when their two CH-53 helicopters crashed off Oahu.

“The decision to suspend the search without finding survivors was particularly difficult,” said Capt. James Jenkins, commander of the Coast Guard’s 14th District.

Searchers discovered some of the wreckage from the helicopters on the ocean floor about two miles offshore in roughly 325 feet of water, Jenkins said at a news conference Tuesday in Hawaii. The Marine Corps will now be in charge of the investigation into what happened and any possible recovery operations, he said.
