Defense News: The Pentagon failed its audit, but officials aren’t surprised
WASHINGTON — The U.S. Department of Defense “failed” its first-ever audit, expected to be released Thursday, according to its No. 2 official.
But Deputy Secretary of Defense Patrick Shanahan isn’t surprised at the result, saying it was widely expected the audit would find issues. The audit, concluded through the DoD’s Office of Inspector General, has long been sought by lawmakers and good-government groups.
“We never thought we were going to pass an audit, right? Everyone was betting against us that we wouldn’t even do the audit,” Shanahan told reporters Thursday at the Pentagon.

Saying the Pentagon "Failed" the audit show a lack of understanding of what an audit is. Audits do not receive grades and/or numeric scores. Audit reports are issued describing the scope of what was audited, and is composed of "Findings", "Recommendations" (of the auditors) and "Management responses" to the auditors recommendations, either disagreeing with the recommendations, or more likely agreeing, along with target dates for implementing the recommendations. The auditees performance in the audit is generally determined by the number of audit "findings" along with their significance.

As an auditor, I always want to find something in order to "earn" my keep.
As an auditor, I always want to break the news gently. I have learned that if you do not, you will be hunted. You let direct reports know of the findings so that they have an action plan to correct the situation so when you provide the report to their supervisor they haver a plan.
Or you will be hunted.
